Forms
The forms below will prove to be helpful when remodeling a house or if you are involved in a construction project. With the exception of the "Contractor Compliance Agreement Form" each of the following forms is required by Florida Law. As such, the format and content are subject to change on an annual basis. Before you use these forms, you should check the statute referenced on the form to ensure the form has not changed.
Miscellaneous Forms
- Contractor's Compliance Agreement: This form will help ensure a contractor complies with applicable laws, regulations and ordinances.
Statutory Notices
- Lender's Warning: If a bank gives the funds directly to a borrower, the bank must provide this warning.
- Mandatory Notices for Construction Contracts: These Notices are required in residential contracts over $2,500.00.
Statutory Forms
- Claim of Lien: This form is to be used to record a claim of lien.
- Complaint for Construction Industry Licensing Board
- Contractor's Final Payment Affidavit: This is the form a contractor must provide to the owner before payment is due.
- Notice of Commencement: This form is required in residential contracts over $2,500.00.
- Notice of Contest of Lien: This form will reduce the time for filing suit to enforce a lien from one year to sixty (60) days.
- Notice of Termination of Notice of Commencement: This form is recorded when a construction project has been completed.
- Notice to Owner: This form is sent to a property owner by a subcontractor or supplier providing goods or services to a construction contractor.
- Partial Release of Claim of Lien: This release is contingent on a check clearing.
- Request for List of Subcontractors and Suppliers: This is the form used to obtain a list of the subcontractors and suppliers for a specific project.
- Request for Sworn Statement of Account: This is a request for the contractor to provide a sworn statement showing how much is owed to subcontractors and suppliers.
- Satisfaction and Release of Lien: This is the form that is recorded to show a lien has been paid.
- Sworn Statement of Account: This is the form used to respond to the Request for Sworn Statement of Account.
- Waiver and Release of Lien Upon Final Payment: This releases all lien rights a subcontractor or supplier may have.
- Waiver and Release of Lien Upon Progress Payment: This releases a subcontractor's or supplier's lien through the date provided for in the release.
